Have to idle truck up to keep it running.
Hesitate in starting

Does it not start right away like an extended crank?

Does it not crank? Please give me as much information as possible.
The idle issue could be a dirty throttle body or a idle air control valve. That is what controls base idle speed and is a very common failure.
