I have had this truck coming up on two years. Since I bought it, I have had to do a heater core (not fun with freezing fingers), and a thermostat twice. But when the weather is cooler outside (even sixty degrees), the truck does not warm up past around 150. The thermostat is fairly new. The hoses feel slightly warm to the touch, and there is very little heat coming out of the vents. I am not getting any check engine codes. I would think that the twenty five minute commute (mostly fifty five mph and above) would get the heat going. Last winter I let the truck idle for fifteen minutes to warm up, temperature gauge barely rose off the bottom end, and no heat. I needed an ice pick to get through it. I do not want my kids to get sick from lack of heat this winter. Please help! Oh, and the mileage I input as the approximate mileage is exactly what the odometer says.

Maybe a stuck thermostat, take it out and see if it is in the open position with a cold engine.

But the temperature gauge shows that the coolant temperature is not even warming up enough to open the thermostat.

Gauge or sender may be faulty. Do a temperature reading with a thermometer in the coolant to get an idea as to what the actual temperature is.

Bad fan clutch? If it is locked it would be pulling a lot of air and causing it to stay cold.
If you grab it with the engine cold does it turn?

When the engine is cold, I tried to turn the fan by hand. It turns, but was pretty stiff. I started the engine, and the fan turned with the engine, not at a reduced pace like one might expect.

That sounds like a bad clutch. It should rotate sort of like you were paddling smooth water. When you start the engine it should turn for a few seconds then start coasting at a lower speed. Then once the engine gets up to temperature it should start to engage and pull air.

The thermostat regulates the engine temperature but if it is just too cold outside at idle the engine may not generate the heat needed and will stay within temperature using just the heater core as the radiator. Some people use block heaters in cold weather does your truck have one?

The engine should heat up no matter what so this is very strange. Try the fan clutch live Steve W has suggested.
