1998 Plymouth Voyager Constantly Overheating

Engine Cooling problem
1998 Plymouth Voyager Front Wheel Drive Automatic 100000 miles

I have done everything I can think of to stop this car from overheating. I've replaced the Thermostat, Replaced the hoses. Drained and refilled the Coolant. What else could be the problem?

Are cooling fans coming on? When it gets warm feel the radiator down towards the bottom is it cooler than the top? If sedament built up in rad. Reducing cooling ability.
