Failed clutch repaired; two days later clutch plate mangled

Honda Accord 1998, 138,000miles, 4 cyl:
While trying to make it back to college, the clutch failed on daughter's honda and dealer replaced w/new clutch disc, pressure plate, bearing and flywheel, assembly transmission and fluid. Checked master and slave cylinder and both were good. Three days later, car makes short banging noises, clutch pops into neutral and car stinks as when clutch failed. She couldn't get it in any gear. Honda dealer close to her college pulls transmission and says clutch plate so totally mangled they can't tell what happened, broke or went wrong, but took pics. Any ideas as to what may have occurred?

If the disc is slightly bent when it is assembled, it will prematurely fail, possibly like what you described. Sounds like this one is on them
