1998 Plymouth Voyager computer codes 13, 43, 45, 66

Truck cuts off, must shift to neutral to keep it running. Trouble codes 13, 43, 45, 66 are indicated from scan.I replaced the O2S-upstream to correct trouble code #66. I don't know what to check/replace for the other above listed codes. My altenator is very noisy, and the belt makes a lot of noise. This van hasn't been driven for 17 months. Could you please help. Thank you.
