1996 Plymouth Voyager Speed Meter Drops to Zero and Gear sw

This problem started month ago, when somethings van runs as normal. But often now van would hesitate to pickup speed after 30mph feels like transmission won't switch gears to 3rd and 4th speed. RPM would run high passing 3000 - 4000 and speed would stay 40mph if press gas to highway speed.

Speed meter would drop to zero when slowing down or hit brakes.

Codes will be 1st to get
cycling the ignition key on off on off on
read codes in the odometer area P 0000

suspect and I would replace input and output sensors on the tranny
