When you start engine it clatters loudly for 2-3 seconds and then smooths out and runs fine. Do you think this is a big problem?

Sounds like a valve lifters leakdown problem-when it pumps up while idling and gets to operating temperature it disappears till next cold starts

Could be a leakdown problem if there are driveability issues. More than likely there are oil passages that have become restrcited over that last 19 years that is keeping the oil from getting up top as fast as it should. It may not be a bad idea to try some chemical in the engine oil (BG) to see if you can free up the startup flow.
