2007 Other Ford Models Clutch Problem

I am trying to find out why my 07 Diesel Ford S-Max has needed 3 clutch replacements in 2 years? Cant be driver error as had previous car for 6 years with no clutch problems? Could it be due to another part on the car damaging the clutch or faulty workmanship by the garage? They say the slave cylinder is also needing replaceing? Hope someone with some knowledge can help :) x

Hi there a few things that come to mind. The line up dowels beetween the engine and box are missing this would rip the centre out of the clutch disc, the crankshaft toe bearing is damaged or missing or if there is a dual mass flywheel fitted it could be faulty.
