138,000 auto trans. Makes an awful noise when I sometimes when I start it up I had the belt and belt tensioner replace it still makes that noise. The noise is on the side where the belt is sounds like its around the Alternator but I can’t tell but, when I raise the up rpms it stops.

Ok, we know what kind of car it is, that's a start. How about now explaining in more detail what the problem is. It makes an awful noise doesn't really tell us much. When does it make this noise, where does it sound like it's coming from? Need to know a little more before we can help.

Sounds like it could be the altenator itself, or the bearings in the pulley. Try pouring some water on the belt around where the noise is coming from and if it stops let me know.
