1996 Chrysler Cirrus LX 2.5 V6 NEED HELP BADLY!

Ok my 1996 Chrysler cirrus lx 2.5 v6 has issues. First of all I replaced the alternator and the TPS. The alternator being changed fixed my high volate at TPS and MAP sensors, but I still have major problems. On an autozone computer test which was free it said fault code p1296 now I can't find this for the life of me. So I basically said forget it. Then I remember I can put my key in the ignition and go on off on off on within a 5 seconds period and it will spit some codes out at me. The codes I got were 12, 14, and 31. What in the world are these and how do I fix my problem? I would really appreciate it if someone could help me I work part-time and go to college full-time.

Daily driving issues are as follows:

I drive the car it runs fine for a while then it shows slight hesitation.
I continue to drive it sometime it acts as if all power fails then it re-picks up idle after about 5-30 seconds or just dies all together leaving me coasting to the side of the road.
I try and restart the car sometimes it restarts, sometimes it starts and dies about 5 times before it goes again.
It's a royal pain in the you know what. Please anybody =help.

Code 31 is an EVAP system problem, code 14 is a MAP sensor circuit failure or bad sensor, and a code 12 is battery disconnect, meaning the battery has recently been disconnected and the computer has not been reset. MAP sensor is easy to replace, but finding the problem with the EVAP system, NOT FUN. Could be vacuum related.

Ok besides the issues I stated before which are kind of irrelevant here is my main problem.

FOund COde p1296 Loss of Reference Voltage to Map Sensor MFG. Specific Code.

Momentary Loss of Map Voltage Signal Under Accel Condition noted once during Approx 16 mile Stop and Go, Drive, car fell Flat for 2 seconds pulling away from stop. Scanner read 0.4 volts during failure.

What shall I do now? I dont have much money im a full time college student and I work full-time. Please help I would appreciate it thank you very much.

Ok I found in a chrysler forum that this is what p1296 means

1296 The MAP sensor doesn't seem to be getting enough electricity.

Why would this be though it seems to happen when the car warms up I do live in arizona temps reach 100+.
