Transmission not shifting properly

Hello,

My transmission (in drive) does shift through the lower gears, but car jerks (usually) through each one and usually not ending in the final gear. I will then drive at about 35 MPH (or less) at 2,500 to 3,000 RPM's, never shifts again.
I will stop, turn off car, then start up again, basically, then the same thing.

Transmission fluid level is okay, not old and mechanic doesn't have OBD1 scanner.

While looking for a scanner, thought to ask you for help and any comments.

Good afternoon,

You can see the codes if you turn the key to the on position and turn it off 3 times. The last time leave the key in the on position. It will flash out codes starting with 11 or 2 flashes slow with a pause in between.
