Car stalls when put in drive

Hello I have a 99 vw polo with a 100,000 miles on it the problem I have is when its started and I put the car in Drive(automatic Trans) it takes a few minutes for the gear to kick in and once it kicks in as I accelerate the car jerks foward

Check the tranny fluid. I would consider changing the fluid and replacing the filter (I think there is one in the tranny on this car).
