Truck wont go more than 20mph

Truck will only go about 20mph. My brother then installed a fuel filter as per auto zone. Nowproblem is wayyyy worse! Goes 20 then down to 5-10 mph with the RPM jumping all over the place. Ran better before filter was put in! Was told before when it was stalling out I needed a MAFS sensor? Could this be the problem? Obviously the filter wasn't bad? And a pain to get to I might add! I'm a single girl and really need help as my brother is not a mechanic. Please. ANY IDEAS WOULD BE APPRECIATED!

Does it smoke? Misfire? Stall out? Or just makes a lot of noise and is slow?
