1994 Toyota Corolla overheating

The gauge reads it is the right temp but steam comes out1st time radiator cap. Checkedit was on crooked. 2nd time through the resevoir. It goes from making extreme heat to no heat. We did flush out the radiator. I just bought the car as is and flushed before this started happening. What can cause these symptoms?

If you flushed radiator and changed thermostat and filled with 50/50 mix of coolant/water.

Next step would be to perform a compression test to rule out headgasket.

If OK, then pressure test cooling system.
