I'm fixing my neighbor's car. I put a new IAC in, it's not the problem. When in Park it starts up kind of have to flutter the pedal a bit to get it going after it had shut off. It seems that it Idles a little high. As soon as I put it in gear Reverse or D it stalls/shuts off. But if I gas it a bit shift at a higher RPMs 2,500 or 3,000 or so it engages, and I can even let off the gas and it stays running. Why is it that it just shuts off when I'm trying to shift like normal? D or P.

It sounds like you need to do a few other things. First remove the throttle body and clean it very well. Now reinstall everything and reset the system by disconnecting the battery for at least 10 minutes. This will erase all learned values. Now start the truck and go for a drive, you may need to play with the throttle a bit to keep it running until it relearns the idle and shift speeds.
