Serpintine belt

Serpentine belt keeps coming off, put new and comes off after couple weeks of driving it.

When the newer belt comes off, is there any sign of wear and tear? Is it coming of intact one piece. Are the pulleys aligned, looking down from top of engine? This would throw the belt. Was the tension-er ever replaced at proper interval?

The belt tension-er's for the drive belts do not really have interval replacements like timing belts I have never seen that in time or mileage maintenance from manufacturers. If the tensions are weak it can throw a belt. Also, what exactly happens like the other used asked? They also sell pulley package kits with belt tensor and idler pulleys.
