Top radiator hose is 162°F the bottom hose is 75°F?

The top radiator hose is 162°F, the bottom hose is 75°F, the heater hose is 158°F and I use my scan tool to check engine temperature, and it read 199°F are theses number acceptable? I believe the heater hoses have been bypassed from the heater core and I can't find the end of the heater core to connect them back up.

Those numbers are okay. If the heater core is bypassed it likely has a leak. I would get a cooling system pressure test kit and adapt it with some hoses to connect to the heater core. Then pressure test it to see if it leaks down. The heater core connections come through the firewall on the right side almost in line with the cylinder head on the engine.

I found where the heater hoses connected to the heater core. How do I replaced the heater core?

Heater core replacement on that truck involves stripping out the dash so that you can get to the HVAC case and remove it, then the core is easy to replace.
