Jumps RPM's and struggles

When I accelerate the rpm's jumps up between three and four and drops back down while I am only driving between twenty five to thirty mph. It also struggles while I am accelerating as if its switching gears on its own. Could it be a transmission problem?

Scan for codes and check fuel pressure with a gauge first auto parts rent it also clean throttle plate on both sides and iac hole with choke cleaner.
