Hybrid battery state of charge falls suddenly from full to two bars?

I have a 2012 Toyota Aqua, when it gets into EV after few meters of driving the SOC (State of Charge) falls suddenly to two bars and the engine revs very high. When it takes distance almost 1 km to add bars but it never goes back to Ev until I shut off and restart again. Please help.

Hi,

First, working on an EV can be dangerous if you are unfamiliar with the procedures, so please be careful.

Also, if you haven't had the can-bus scanned, I would recommend it. There could be diagnostic trouble codes stored that are helpful.

As far as the issue, I attached diagnostics below which are general for the circuit. Take a look through them and let me know if you are comfortable performing them.
