1997 Dodge Dakota check engine light came on

Check engine light came on so I checked codes by holding the odometer button and cycled key on and off 5 times and it displayed a number 12. What does this mean and does anyone have any advice to fix this problem? And while speeding up to enter the freeway it will pulse or cut out at around 45 mph but ounce up to speed it is fine, but if lets say I want to accelerate from 65 to 75 it pulses and cuts out again. This all started about a week ago when I started it one morning and it was missing out real bad until it warmed up and then my check engin light came.

When the last time it was tune-up? As per the CEL take in to Autozone they'll pull the code/s free of charge this is your starting point finding out what's going on in the engine management system
