Common cause is the ignition module. But it can also be caused by coil #4 open circuit to the PCM or shorted to ground.

To check for on open in the harness between PCM and IM:

Key off.
Disconnect ICM harness connector (Pins 1-6).
Disconnect Powertrain Control Module (PCM) connector. Inspect for damaged or pushed out pins, corrosion, loose wires, etc. Service as required.
Measure resistance between DPI Pin (#6 in connector) at the ICM vehicle harness connector and Pin 32 at the PCM connector.
Is resistance less than 5.0 ohms? If NO, then there is an open in the circuit.

You also will want to remove and clean under the mounting screws for the DIS module, it grounds through them, any corrosion can cause problems.
