I must warm my car for 3-5 minutes before driving, otherwise the engine will stall many times before continuing. It will idle fine once started but if I begin to drive it will stall at least once and usually twice. This problem goes away once the car is warmed up.

Will it stay running if you hold the gas pedal down a little? If so, has the battery recently been disconnected or run dead? If it has, the memory was lost in the engine computer and it must relearn "minimum throttle". To do this, drive at highway speed with the engine warmed up, then coast for at least seven seconds without touching the brake or gas pedals.
