2003 Chevy Silverado feel heat but blower is not working

When I am driving my truck. I feel heat by the vent but the blower is not working. For awhile it was not working on setting number 5 (high) but it worked on 1 - 4 settings. Tonight it is 9 degrees and the fan did not work at all. Any suggestions welcome.

HAVE YOU CHECKED THE FUSE/S/RELAY? MAY BE THE BLOWER SWITCH OR RESISTER.
