MAF, MAP, Lean bank1, lean bank2, Misfire?

I am having occasional starting problems and I get these codes: P0101 (MAF range), P0108(MAP circuit high input) P0171(lean bank 1) P0174 (lean bank 2) and P1351 (man. Controlled ign. System or misfire.
- I have already cleaned the elements in the MAF and checked all connectors. The truck will not start at all sometimes and then start right up after sitting a week. Could one problem cause ALL this or would it be multiple problems?

I suspect we have a vacuum leak that is causing an issue when it is hot or still has fuel in the engine. When the engine sits for a week, the PCM dumps enough fuel to get the engine to start.

However, we need to start with checking for a vacuum leak which will cause a lean condition.

If there are no leaks, then we need to find out if we have low fuel pressure or some other reason which means we need to check the pressure.
