My truck is leaking radiator fluid in the #4 cylinder, is that a blown head gasket, and if it is how much does it cost to repair.

It may be a blown headgasket, but first get a cylinder compression test performed on teh cylinders and if the #4 cylinder is lower then the rest the gasket is probably bad, if its good and a cylinder leak down test whos good results, then I would suspect a intake manifold leak.

That 2.2 engine is fairly common for a head gasket leak at number 4 cylinder anywhere from 100,000 miles and on. Bubbles in the rad or a leakdown test are the best indicators.
