Air flow does not come out of the mid level?

Your issue will be with either the control unit or the mode blend door. The only way to tell for sure is to test both. You'll need to remove both for testing. The control unit will require access to the specs/values of the circuit board in order to probe the contacts/wires, which I don't have access to.

For the blend door, it is easiest to just remove the blend door and with I plugged in work the control to se if it moves if not replace it here are the instructions in the images below, I can tell you nine times out of ten it will be the blend door actuator.
