Bad mechanic

I put my car in the shop for a radiator repair and when I got it back up and running really rough. When I pop the head my serpentine pills snapped my pulley broke. I looked at my fan collection the bolts were not even put in all the way. Did that caused the problem?

I am not completely sure what bolts were left loose. Is it possible for you to upload a picture of the damage as well as the bolts you are referring to?
