Bad idle and rough jumpy acceleration.

I have a d16y8 in my 96 5 speed civic. About a month or two ago the rpms jump up n down at idle or if my foot presses the clutch. And after it gets good and warm it has a very rough acceleration with a tiny amount of pressure to the gas it takes off wide open, if I let off the gas it's jerks and bogs down. I need help it's my daily to work
