Air conditioner is only blowing out the floor vents

My air conditioner only blows on floor. I need help diagnosis the problem.

There is a mode door actuator on the left side of the HVAC housing that can be accessed from under the dash on the driver's side. The actuator could be bad or the mechanism could be jammed.
