Overheating

It is slowly leaking antifreeze towards the back end of the engine bay. Temperature gauge needle moves towards the H while parked with A/C on, also while driving (A/C can be on or off).
If car is parked with A/C off, temperature gauge needle slowly makes its way back to middle from H, but struggles and maintains between mid temperature area and H. Radiator fan turns on and works. Any suggestions on why it could be leaking and causing the temperature to not stay at a moderate stage?

If it is in the rear of the engine, chances are it is a heater hose. There are two hoses that will run from the engine to the firewall under the hood. Follow them and inspect. To do this correctly, the engine should be cool and you should pressurize the system. Take a look through these links.
