Runs rough

When first started it runs very rough and sounds like it is going to cut out. When I hit the gas pedal it makes it worse. Also it smells like fuel. The engine size is 4.3L

How long since a tune-up? Any applicable trouble codes? You should be able to check mil codes, yourself. Check for external gas leak. Check vacuum line to fuel pressure regulator. Check evap system.

I think it needs a fuel filter do not see any gas leaks

If you suspect a fuel issue, use a gage and check fuel pressure.

I will try that after would a exhaust manifold cause that problem
