92 Toyota Pickup - Clutch or Transmission

My son just brought home a 92 4x4 pickup, 200,000 miles. Help!

Can't get into gear while the engine is running - lots of effort and/or grinding. When engine is off, can get into gear no problem.

Repair shop just said 'replace the transmission' - how can I tell if it's the trans or the clutch?

Bleed the clutch slave/servo, adjust the free play and pedal height-this don't cut it-time for a new clutch assembly that means clutch disc, pressure plate, release bearing and pilot bearing
