Turn Signal Problems - Grand Caravan

I have a 2002 Dodge Grand Caravan with 135,000 miles. The turn signals are very eradic. Sometimes they work, sometimes only one side works, sometimes I turn the left turn signal on and the right turn signal is activated and vise versa. Sometimes they don't work at all. Occasionally, I can flip the turn signal arm up and down a few times and it corrects the problem, but not always?

Any ideas?
