1990 Dodge Caravan serpentine belt

Whenever I drive in wet conditions, the serpentine belt comes off the pulley system. I have had the belt replaced once in April and have had to have the belt reinstalled twice since that time.

Was the adjuster replaced also, it maintains tension in all conditions, and there needs to be protection from the elements(shield) and proper alignment. Is any accessory siezing intermittantly? Or pulley out of alignment? Wrong belt?
It just doesn't slip from design flaw. Nice van. Let me know, I am Joe, more experience with other things on your van but I have worked with this problem a couple dozen times. Hope you can fix it. OK What engine what state, what is the wet condition? Not normal. It does happen. I'll check back later or tomorrow.

Replacing the belt tensioner should solve the problem
make sure the correct belt size was used
