This codes is not widely known, can I ask how you got it and if there are any other issues you are having with the car? Some say it is a coolant sensor for the hybrid system so I would check the EV battery coolant level first and if low refill and clear the codes. Also, inspect the system to see if there are any leaks. I also found that it could be the Exhaust Gas Heat Recovery sensor or the regular engine coolant sensor if the car is a plug in hybrid or not. There is a coolant sensor for the hybrid motor and battery as well. Are there any other codes? You might need Ford-Specific Diagnostic Tool to tell which sensor it is. Please let me know what you find, I am interested.

Went to AutoZone and checked it on mine.
I clear it and it’s good for one cycle then pops back on.

Yep, that is the code alright, I don't know which coolant sensor would be #4. You will need to have Ford scan it for more information. Sorry I was not more help.
