Hello,

This sounds like it could be a problem with the defrost actuator. This actuator is responsible for routing the air to defrost, vents, or floor ducts. It, like everything else, wears out from normal wear and tear over time, and when it fails, the door will be stuck routing the air to whatever position it is in when it fails. I have included a link for you down below to go to.

As far as the vacuum hose you are probably going to have to get it from underneath the dash and re route it through the hole in the firewall it originally went through. I have included the vacuum diagram you requested in the diagrams down below. If you need step by step instructions on how to replace your vehicle's defrost actuator or disassembly of the dash to get the vacuum hose please don't hesitate to ask and we will get that right to you. We are here to help.

Thanks,
Alex
