I have the Starter mounting bolts and wires removed, how do I get it out of there? There doesn't seem to be room to pull it out, I've twisted and turned it but I don't see a path for it to come out.

You will need to unbolt the exhaust from the manifolds to gain enough room to get the starter out.

Does it come out from the bottom of the truck?

Well I did find a better way. I remove the passanger tire and then remove the splash guard between the frame and the body. Now you can see the cables unbolt them (make sure battery is disconnected). Now unbolt the starter the lower bolt will have to be done from under the truck then twist and turn and bring the start through the hole that you have with the splash guard removed. Hope this helps better then dropping the exhaust.
