1992 Toyota Celica Lean Air/Fuel fault code, engine hesitat

1992 Toyota Celica 4 cyl Front Wheel Drive Automatic

My check engine light comes on around 55 mph and goes off when I stop, it's throwing a code 25 (lean air/fuel mixture) I just got a full tune up a month ago, I have cleaned the IAC, replaced the TPS, My fuel pump is good. When I accelerate, my car also hesitates, I can push the pedal all the way to the floor, and it takes a second for it to respond, and when it does respond, it still doesn't have the power that it should have. What could be causing this?

Lean fuel condition could be caused by one of the following below:

1. Vacuum leaks
2. EGR and PCV valve.
3. Low fuel pressure.
4. Dirty fuel injectors.
5. Mass Air Flow Sensor.
