Grinding in between gears

1994 Saturn, high mileage. Everytime I shift it grinds going into every gear. But when I put it into neutral and wait a few seconds before going into another gear it goes into gear perfect? I have exhausted my ideas for this problem and am unsure on what the problem is. Thank you in advance for your input!

There are bushings at both ends of the shift cable that wearout. This can cause problems with going into gear. Replace the cable.
