Rough Shifting/ Hesitation when Accelerating?

Every time it shifts on normal acceleration it shakes hard. It hesitates to go when accelerating, just giving it a little gas to try and pass someone. The RPMs also bounce about 100-200 constantly while steadily cruising. This has been happening for about 2 months but seems to be getting a little worse over time and the shakes are getting rougher.

I just did a fresh oil and oil filter change, new air filter, replaced all of the spark plugs and cleaned the throttle body. (Still has the same issues)
No check engine light until about a few days ago, the code is P219A. I'm not sure if it's related or if that's something new.

The P219A code is the clue because there is an imbalance between the cylinder banks which means there is an engine misfire which is causing the problems, I would start with a good tune up including spark plugs and throttle body service. Here are generic guides to help you see what you are in for when doing the job and instructions in the images below to show you how on your truck. Once completed clear the codes to see what happens.

Thanks for the reply, I recently replaced all 16 plugs and cleaned the throttle body about 3 days ago. I cleared the code and it came back up a day later. Even after that tune up it doesn't feel like anything has changed at all.

Okay, when it cranks over does is sounds normal? Is the engine misfiring? Can you please shoot a quick video with your phone so I can see the engine running? That way I can tell for sure what's going on. You can upload it here with your response.
