Rough idle and multiple codes

I did notice some things on your live scan data, everything I just typed out for erased, so here it is again.
1. The Coolant temperature sensor is only reading (pics 1 and 2, 3) and the cooling fan jumped from 10% to 54.4% and the car is nowhere near operating temperature.
2. Outside Air Temperature Sensor is reading 419 F (pic 4).
3. And I'm not sure about this model, but I don't think the camshaft should be advanced 6.25 degrees at idle, but I could be wrong about that one.
So, there's some other strange stuff going on here, also on that third picture, the specified actual temperature is looking for 194 degrees, so the ECM knows it should be that hot, but only reading 94 degrees,
Did you have two 5-volt reference wires at the APP sensor on the gas pedal? I'm wondering if you have a low reference voltage and that's why the coolant temperature sensor is reading incorrectly, but strange you have no codes for anything like that. The +99% on the 02 sensor Bank 1 sensor 2 is odd as well, if that's correct and the vehicle is positive on its fuel trims, so adding fuel, which will point to false air/ vacuum leak issues.
Keep an eye on that coolant temperature tomorrow and see if it goes above 94 f.

I could not get access to pedal sensor. I will need to remove few things. Yes, I noticed few weird readings just wanted to make sure, bit am sure all related. I will check the coolant temperature.

I was mainly concerned you had a full 5-volt reference. So, any sensor that's uses it should do.
Youve found your main issue though, the smoke machine was quite obvious. You'll have to check into that area for a leak.

Could you suggest anything with regards to the electric fan being always on since car is started cold? It shits off when I shut the car off. But it doesn't make sense that it is always on at same high speed as soon as I start the car. No leaks of coolant and tempurature gauge is where it should be.

Just found out the ambient temperature sensor isn't even there, harness is just hanging. Not sure of that does anything other than read the outside temperature, if it will actually contribute at all to the fans being on all the time, both of them.

I also cleaned the bottom of the engine and top to see if there are any leaks and nothing is dripping. Added some at-205 reseal in case the rear main seal is leaking, needs to run for few hours, but its already running smoother being parked and running for 2 hours. Will see how it goes. Here are few numbers while running.

Just be sure we are keeping each post to the same issue. The fan issue is one we need to start a new post on if that is still an issue as it would not be related to the rough running and others would benefit from what you find on it.
