1991 Plymouth Voyager driver side axle breaks

My driver side axle keep going out it pulls apart on the motor side what could be the reason this keeps happening I have replaced it many times and it keeps doing the same thing in the same spot and it dont matter how much you gas it the motor and trans mounts are new so I know it cant be because of the motor or trans shifting around hope you can shead some light here because im in the dark at this point

Typically two things cause this, a broken transmission mount and two, the engine not being centered after the mounts are replaced. Loosen the mounts back up, and follow this for the specs. Pry the ninge to the point that the shafts measure correct and then tighten the mounts back down.
