Oxygen sensor replaced three times in one month

A month ago I replaced my upstream oxygen sensor due to check engine light. Two weeks ago the check engine light came on again, and the same o2 sensor was replaced under warranty. Yesterday, the check engine light came on again and it was the same O2 sensor again. Did I get two defective o2 sensors like my mechanic said, or could there be something else causing these new O2 sensors to keep going bad?

What brand of O2 sensor was put in it? Toyota's are particular about only having Denso parts installed especially when it comes to O2 Sensors. Also make sure wiring from ECM to sensor is ok. Thoroughly check harness.

Do you have any exhaust leaks near the 02? Is the harness chafed or broken anywhere?How does the engine run?

Thank you very much for your quick response. I don't know if my mechanic used a Denso O2 sensor. I will take your response to my mechanic. I hope it will help him determine what needs to be done so that I won't have to continue going back to him to the same O2 sensor replaced again and again. After paying for the first replacement, the additionals are covered under warranty, but it still is a time consuming nuisance.
