My brakes are grinding when I come to a complete stop. What's wrong with my brakes?

One or more of your brake pads have broken of the mounting plate. Now the mounting plate is touching metal to metal to your brake rotor.

You have to replace the pads (for sure) and possibly able to turn the rotor, if not replace.
