2002 Nissan Altima head gasket?

My car started overheating about 6 months ago. I've had it serviced three times and all three times it worked fine for 1-2 months. Now I have the creamy look in my oil and I am losing coolant regularly. When engine is running and radiator cap is off when you rev engine water sprays out through cap? HELP

Sounds to me utmost like a blownhead gasket or a cracked/warped head due to the overheating.
