1998 Mercury Sable I think its the transmission

While i'm driving and coming to a stop, while the car is in drive, it starts to rev up or sometimes it will act like its in neutral. It also happens when I put the car in park. It just revs up. I haven't done any repairs to the engine or transmission. Just bought the car 4months ago and it was not working like this before. It has fluid in the transmission, correct levels. Was wondering with your expertise knowledge if you may know what could be causing this. Thanks.

Suspect idle air valve
check for vacuum leaks
and check motor and tranny mounts

Even when im driving and the light turns red, I come to a stop, I can feel the transmission like changing gears. When the like turns green, still in drive, I hit the gas and revs like in neutral. This can possible be caused by idle air valve, vacuum leaks and motor and tranny mounts? Could there be something in the transmission that has to be replaced?
