Lug Nut Covers Missing

My front passenger tire had an issue 4 years ago when all 5 lug nuts snapped and the wheel popped off during a left turn. Recently, I've heard a slight grinding noise from that area when I turn the wheel so I looked at the tire and noticed that 4 of my 5 lug nuts were missing caps. Is this dangerous? Would I be able to fix it myself or would I need to take it in? Would it be an extensive fix?

It may be a wheel bearing problem which would mean to take brakes off and axle loosened to get wheel bearing out but you have to make sure it's the right side. The cap thing you could do they just screw on over lug nuts.
