1996 Lincoln Continental heater core

My front window defrost is slow and not working as good as it has in the past. And also my rear door windows are staying fogged over with a thin layer of frost that won't go away. Heater core? Is it that hard to replace for someone that knows a thing or two? Thanks for your time.
