Hissing noise along with rough idle

For the past five months or so it has had a really bad idle. It idles at almost 0 rpm's. Along with the low idle it has a really bad sputter, and has trouble when taking off from a stop (I am thinking a misfire). I have been driving it like that for nearly five months, as no one can pin point the problem, and I have replaced many things already. Nothings seemed to change it. Just today it started making a hissing nose under the engine, sounds almost like air being brought in. Any idea what this could be, and if they are related?

The noise sounds like a vacuum leak which can effect the engine running cause a miss etc. Has anyone ever done a smoke test looking for a vacuum leak?

The transmission just totally went, after barely making it home. Not sure if it was related to the noises, but now it is a much worse problem.
