1990 GMC Sierra Smoke

When I frist start my truck after beeen setting for a while a puff of blue smoke come out the tailpipe and then that it. It don't smoke when i'm driving and there is no oil in the tail pipe, just a puff of smoke on the start up.

With 270000 miles, a blue puff of smoke at start up is normal. Oil probably leaks down from the valve seals after the engine is shut down, causing a little oil to sneak into the combustion chamber. Unless you are consuming a lot of oil, meaning your oil is low all the time -- I wouldn't worry about it. Unless, of course, you want to rebuild your engine? Engines do not last forever - and 270000 miles on V8 is a good run.
